Print out the form and have your teachers check the box and sign on the line and have your parents sign the bottom of the form.

Each member of the team will remain on the team for about 1 month and learn at least 2 different positions on the team. All students on the WWTV team will have to arrive at 7:30 A.M. Please sign below giving your approval for their participation.
